In order to make your visit more efficient, we’ve included some useful forms that you can download and prepare in advance of your appointment. Please bring your completed forms to Scottsdale or Phoenix Dermatology at the time of your appointment. Our friendly registration staff will take it from there.

 You will need to fill out a few forms on your first visit including a patient registration form and a medical history form. For your convenience, you can download and print these forms located below .
Please fill out all information to the best of your knowledge. Please bring your insurance card to your appointment. If your insurance requires a referral and you have received it, please bring a copy of the referral. We must have a copy of your insurance card in order to verify benefits and be prepared for your visit.
